PATH Training (STEP)

Standardized Team-Education Program

Interdisciplinary team-based collaboration is central to the PATH program and achieving true collaboration demands that each team member carefully reflect on his/her role and practices.

STEP is an ambition program of skill building and team re-organization for improved efficiency to:

1. Eliminate repetitive assessments

2. Improve the relevance of the patient assessment

3. Optimize team communication

4. Make better use of the assessment for care planning

5. Improve system navigation

Demand for STEP training is growing across Canada. Currently, teams in four provinces have received training.  Research remains integral to improvement of PATH, therefore training is offered to team who collect data from which to better understand the clinical, administrative, and economic implications of the program.
